Enrollment Forms

Please print and complete the following forms and submit them to the Great Escape Program Directors, along with the non-refundable $150/child, $200/family Enrollment Fee.

Submitting the Enrollment Forms and Fees serves as your reservation in your child’s program. If space is not available, your child will be placed on our waitlist in the order that the Enrollment Form and Fee was received.
